Use the following information below to request records under the PA: Requests for C&P exams, C-Files, VBA Benefit Records must be submitted via mail or fax to Department of Veterans Affairs, Evidence Intake Center, PO Box 4444, Janesville, WI 53547-4444; Fax: (844) 531-7818 or you are allowed to submit a request for claims information electronically to AccessVA . To avoid processing delays, upload a signed SF 180 or VA Form 20-10206 . Requests for VHA Health Records must be submitted to https://www.va.gov/health-care/get-medical-records Subrogation Requests must be submitted to https://www.va.gov/OGC/Collections.asp
